Persoonlijk begeleider, Groenstraat 110 B Tilburg Wil jij werken met mensen met een licht verstandelijke beperking en hen ondersteunen bij het wonen en leven in een middelgrote stad? Voor de Groenstraat 110-B in Tilburg zijn wij op zoek naar een betrokken persoonlijk begeleider. Je biedt duidelijkheid, denkt in mogelijkheden en helpt bewoners om hun dagelijks leven zo zelfstandig mogelijk vorm te geven. Wat ga je doen? Als persoonlijk begeleider ben je voor meerdere bewoners het vaste aanspreekpunt. Je leert de bewoners goed kennen en kijkt samen met hen naar wat zij nodig hebben om prettig en zo zelfstandig mogelijk te wonen en leven. Het wonen en werken in een middelgrote stad kan voor onze bewoners best een uitdaging zijn. Jij helpt hen om hun dag door te komen, keuzes te maken en om te gaan met situaties die zij tegenkomen. Samen met de bewoner stel je een ondersteuningsplan op. Je houdt dit plan actueel, ziet toe op de uitvoering en kijkt regelmatig of de begeleiding nog aansluit bij de doelen en behoeften van de bewoner. Je observeert, signaleert veranderingen en bespreekt wat je ziet met de bewoner en je collega's. Daarbij werk je vanuit de duidelijke visie van ASVZ en kijk je steeds naar wat iemand wél kan. Je werkt op verschillende momenten van de dag. Hierdoor is geen dag hetzelfde en krijg je te maken met verschillende situaties en ondersteuningsvragen. Jij weet daarin rust en duidelijkheid te bieden en blijft betrokken, ook wanneer het even tegenzit. Je werkt in een klein team van vijf medewerkers. Samen zorgen jullie voor de dagelijkse begeleiding en een prettige woonomgeving. Je stemt goed af, communiceert duidelijk en kunt op je collega's rekenen. Tegelijkertijd neem je zelfstandig verantwoordelijkheid voor de bewoners waarvoor jij persoonlijk begeleider bent. Wie zoeken we?
