Hier werk je aan herstel én veiligheid

Het Ambulant Centrum Rotterdam biedt forensische en intensieve (outreachende) psychiatrische zorg aan volwassenen. Je komt te werken in het forensisch FACT-team, een betrokken en multidisciplinair team dat zich specifiek richt op ambulante forensische verslavingsbehandeling. De patiënten die wij behandelen kampen met (dreigend) normoverschrijdend gedrag en ernstige psychiatrische aandoeningen, vaak in combinatie met verslaving, persoonlijkheidsproblematiek en maatschappelijke uitval.

We bieden zorg op maat waarbij behandeling van risico- en delictgedrag hand in hand gaat met het versterken van herstel op alle levensgebieden. Denk aan wonen, werk, relaties of zingeving. Onze patiënten bevinden zich zowel vrijwillig als in een (justitieel) gedwongen kader in zorg.

Jouw rol als behandelaar

  • Je werkt outreachend én poliklinisch: je ziet cliënten thuis, op straat of bij instanties – waar de hulp het hardst nodig is. Voor outreachende afspraken kun je gebruikmaken van onze dienstauto's en elektrische dienstfietsen.
  • Je coördineert de zorg rondom jouw cliënten en schakelt met hun netwerk, verwijzers, collega’s en externe partners.
  • Je denkt multidisciplinair en systemisch: samen met psychiaters, psychologen, verpleegkundigen en maatschappelijk werkers geef je vorm aan het behandelplan.
  • Je zet een breed scala aan interventies in: van diagnostiek, begeleidende gesprekken en crisisinterventie tot motiverende gespreksvoering, herstelondersteuning en risicomanagement.
  • Je werkt nauw samen met ketenpartners zoals reclassering, gemeenten, woningcorporaties, verslavingszorg en maatschappelijke opvang.

Wat breng jij mee?

  • Een afgeronde hbo-opleiding (bijv. SPH, Social Work met uitstroomprofiel Zorg of een registratie als GGZ-agoog, MWD, of HBO-V met geldige BIG-registratie).
  • Je hebt ervaring (of affiniteit) met psychiatrie, forensische zorg en verslavingsbehandeling.
  • Je bent stressbestendig, flexibel en weet verbinding te maken, ook in de meest ingewikkelde situaties.
  • Je hebt een rijbewijs omdat jouw cliënten zich verspreid in de regio Rotterdam bevinden.

  • Salaris conform cao ggz (FWG 55), max. € 5.294,- bruto o.b.v. 36 uur.
