Als maatschappelijk werker bij Fivoor werk je met een bijzondere doelgroep: patiënten met een licht verstandelijke beperking (LVB), psychiatrische aandoeningen én een forensische titel. Je bent de schakel tussen de patiënt, diens omgeving en betrokken ketenpartners. Een betekenisvolle functie waarin je jouw expertise inzet om bij te dragen aan behandeling en perspectief. Je werkt op de klinieken in Den Dolder, vlak bij Utrecht, binnen Wier+ en de gesloten kliniek. Binnen het team van Wier is vacature ruimte ontstaan door interne verschuivingen.

Je maakt als maatschappelijk werker deel uit van een multidisciplinair behandelteam waarin jouw sociaal-maatschappelijke expertise een belangrijke plek inneemt. Het maatschappelijk werkteam is klein en toegankelijk, met veel ruimte voor eigen initiatief, professionele autonomie en het meedenken over de ontwikkeling van de zorg. We waarderen jouw ideeën en inbreng en geven je de ruimte om actief bij te dragen aan verbeteringen binnen de zorg en het team.

Dit ga jij doen

Als maatschappelijk werker bij Wier:

  • Regel je praktische zaken zoals huisvesting, financiën en verzekeringen
  • Ondersteun je bij aanvragen voor bewindvoering, mentorschap en curatele
  • Heb je kennis van de verschillende financieringsstromingen
  • Onderhoud je contact met familie, justitie, gemeenten en andere ketenpartners
  • Vertaal je behandeldoelen naar een maatschappelijk werkplan
  • Werk je samen in een multidisciplinair team en neem je deel aan overleggen en evaluaties

Hier kom jij te werken

Je komt te werken binnen behandelcentrum Wier. Hier behandelen we mensen met een licht verstandelijke beperking (IQ 50-85), vaak in combinatie met psychiatrische en gedragsproblematiek. De zorg is hoogspecialistisch en vindt zowel klinisch als ambulant plaats. Je maakt deel uit van een hecht, multidisciplinair team waar collega’s elkaar aanvullen en versterken. Als maatschappelijk werker speel je daarbij een sleutelrol: jij verbindt patiënt, systeem en ketenpartners. Samen zorgen we voor perspectief én veiligheid. De nadruk van de functie ligt bij de klinieken Wier+ en Wier Kliniek Gesloten in Den Dolder vlak bij Utrecht.

Dit ben jij

  • Je hebt een hbo-diploma Social Work (zorgprofiel), Maatschappelijk Werk of Sociaal Juridische Dienstverlening met aanvullend de GGZ-agoog en de daarbij behorende registratie en voldoet met je diploma’s aan de criteria van het Zorgprestatiemodel
  • Je hebt ervaring of affiniteit met de (forensische) psychiatrie, LVB-doelgroep of verslavingszorg
  • Je hebt actuele kennis van wet- en regelgeving binnen het sociaal domein
  • Je bent zelfstandig én collegiaal ingesteld, met heldere communicatie
  • Je kunt goed samenwerken met collega’s intern en partners extern
  • Je behoudt het overzicht over de sociaal maatschappelijk aspecten van de behandeling en kan aansturen waar nodig
  • Je staat stevig in je schoenen en beschikt over doorzettingsvermogen met een gezonde dosis humor
  • Je bent 28-36 uur per week beschikbaar.

Dit krijg jij van ons

Wij bieden jou een contract volgens cao ggz voor bepaalde tijd met uitzicht op een vaste aanstelling. Deze functie is ingedeeld in functiegroep:

  • FWG 55 met een maximaal salaris van €5.294,- bruto per maand o.b.v. 36-urige werkweek.
