Als klinisch casemanager speel jij een sleutelrol in het behandeltraject van onze patiënten. Op de Forensisch Psychiatrische Afdeling (FPA) in Den Haag combineer je twee rollen. Je behandelt een eigen caseload én ondersteunt de regiebehandelaren. Je schakelt moeiteloos tussen patiëntcontact, risicotaxaties, overleg en coördinatie. Daarmee lever je een belangrijke bijdrage aan veilige en effectieve behandeling van patiënten met complexe psychiatrische en forensische problematiek.

Dit ga jij doen

Geen dag verloopt hetzelfde. Je werkt nauw samen met patiënten, behandelaren en het netwerk rondom de patiënt.

  • Je voert behandelgesprekken met patiënten uit je eigen caseload.
  • Je neemt deel aan behandelgroepen, multidisciplinaire overleggen en patiëntbesprekingen.
  • Je onderhoudt contact met familie, verwijzers en andere betrokken partijen.
  • Je begeleidt therapievaardigheidstrainingen.
  • Je bereidt zorgmachtigingen voor en zorgt voor een zorgvuldige terugkoppeling van patiëntbesprekingen.
  • Je ondersteunt regiebehandelaren met verslaglegging, voorbereiding van behandelingen en risicotaxaties.
  • Je analyseert complexe situaties en vertaalt deze naar passende behandeladviezen.
  • Je stemt dagelijks af met collega's om behandeling en veiligheid optimaal te waarborgen.

Hier kom jij te werken

De FPA in Den Haag biedt behandeling aan patiënten met complexe psychiatrische problematiek, vaak gecombineerd met forensische risico's. Binnen deze dynamische omgeving werk je samen met verpleegkundigen, behandelaren, regiebehandelaren en andere disciplines.

Je krijgt veel verantwoordelijkheid en werkt op het snijvlak van behandeling, veiligheid en herstel. Dat vraagt om vakmanschap, scherpte en samenwerking. Juist die combinatie maakt het werk uitdagend en betekenisvol. Samen zorgen jullie voor een veilige behandelomgeving waarin ontwikkeling centraal staat.

Dit ben jij

  • Je hebt een afgeronde hbo-opleiding die declarabel is binnen het Zorgprestatiemodel, zoals hbo-verpleegkunde, social work (uitstroomprofiel zorg) of ggz-agoog.
  • Je hebt ervaring binnen de forensische zorg of forensische hulpverlening.
  • Je hebt kennis van psychiatrische ziektebeelden, ontwikkelingsstoornissen en het Zorgprestatiemodel.
  • Je hebt ervaring met registratiesystemen en risicotaxatie-instrumenten.
  • Je hebt aantoonbare kennis van behandelmethodieken en behandeltheorieën.

Dit krijg jij van ons

Wij bieden jou een contract volgens cao ggz voor bepaalde tijd met uitzicht op een vaste aanstelling. Deze functie is ingedeeld in functiegroep:

  • FWG 55 met een maximaal salaris van € 5294,- bruto per maand o.b.v. 36-urige werkweek.
