Ben jij een doortastende coördinator met oog voor detail? De functie van klinisch casemanager binnen onze AFZ in Poortugaal is een uitdagende en verantwoordelijke functie. In deze functie coördineer je behandeltrajecten van begin tot eind en zorg je ervoor dat de patiëntenzorg optimaal wordt vormgegeven.

Dit ga jij doen

Als klinisch casemanager speel jij een sleutelrol in het behandeltraject van onze patiënten. Samen met de regiebehandelaar en persoonlijk begeleiders coördineer je de zorg vanaf instroom tot uitstroom. Jij zorgt ervoor dat alle collega's nauw samenwerken en dat de patiëntenzorg optimaal wordt vormgegeven. Je bent verantwoordelijk voor het patiëntendossier, inclusief behandelplannen, ontslagbrieven en woonprofielen.

Daarnaast heb je contact met patiënten in het kader van hun behandeling. Je werkt intensief samen met hun sociale netwerk en met externe en interne partners, zoals reclassering, ambulante behandelaren, casemanagers en penitentiaire inrichtingen.

Hier kom jij te werken

De Afdeling Forensische Zorg (AFZ) biedt een vaste structuur aan de opgenomen patiënten (mannen en vrouwen) waarbij behandeling en begeleiding in het teken staat van herstel en terugdringen van de recidive in delict gedrag. De AFZ is een gesloten beveiligingsniveau 1 afdeling die bestaat uit drie afdelingen met zowel langdurende (tot maximaal een jaar) als kortdurende (maximaal 3 maanden) opnames van patiënten. Ook beschikt de AFZ over een Detox afdeling met 6 bedden.

Dit ben jij

  • Een afgeronde hbo-opleiding die declarabel is binnen het Zorgprestatiemodel (zoals hbo-verpleegkunde of social work met uitstroomprofiel zorg of ggz-agoog);
  • Een rijbewijs B (een eigen auto is handig, maar je kunt ook gebruikmaken van een auto van de kliniek);
  • Affiniteit met (forensische) psychiatrie en/of verslavingszorg;
  • Een stevige persoonlijkheid, die makkelijk in gesprek gaat, geduld heeft en doorzettingsvermogen toont.

Wij bieden jou een contract volgens cao ggz voor bepaalde tijd met uitzicht op een vaste aanstelling. Deze functie is ingedeeld in functiegroep:

  • FWG 55 met een maximaal salaris van €5.294,- bruto per maand o.b.v. 36-urige werkweek.
