Wil jij werken met cliënten die intensieve begeleiding nodig hebben en zoek je een rol waarin je echt impact maakt? Op onze Beschermd Wonen locatie in Beekbergen begeleid je mensen met complexe psychiatrische problematiek in een setting waar structuur, nabijheid en samenwerking centraal staan.

Wat bieden wij jou

  • Een dienstverband van 36 uur per week
  • Salaris conform [job.schaal_+_salaris] bruto per maand (o.b.v. 36 uur p/wk)

Dit is je team

Je komt te werken op onze locatie Beschermd Wonen op het terrein Het Hoogeland van IrisZorg, aan de rand van Beekbergen en Apeldoorn.

Samen met je collega’s begeleid je 13 cliënten met een intensieve begeleidingsbehoefte en complexe, vaak gecombineerde psychiatrische problematiek. Ook is er één BOR-bed en bieden we telefonische bereikbaarheid voor cliënten die zelfstandig wonen.

Wat ons team kenmerkt? We werken met een open deur en veel onderling vertrouwen. We bieden structuur, nabijheid en duidelijke kaders, maar blijven altijd in samenwerking met de cliënt. Dat maakt het uitdagend, dynamisch en betekenisvol werk.

Wat ga je doen?

Je begeleidt cliënten op weg naar herstel en een zo zelfstandig mogelijk leven, waarbij je steeds aansluit op wat iemand nodig heeft.

  • Je stelt samen met de cliënt een woonbegeleidingsplan op en coördineert de uitvoering
  • Je werkt herstelgericht en stimuleert eigen regie en zelfstandigheid
  • Je biedt structuur, nabijheid en duidelijke grenzen waar nodig
  • Je ondersteunt bij dagelijkse taken en maatschappelijke rollen
  • Je biedt sociale en emotionele ondersteuning en helpt bij praktische vraagstukken
  • Je schat in wanneer begeleiding opgeschaald of afgeschaald moet worden
  • Je ondersteunt bij het opbouwen en onderhouden van een sociaal netwerk
  • Je helpt cliënten bij het vinden en behouden van passende dagbesteding
  • Je onderhoudt contact met netwerkpartners zoals familie, huisarts en andere zorgaanbieders

Dit vragen we van jou

  • Je hebt een SPW, MMZ of andere relevante agogische opleiding op mbo 4 niveau
  • Je hebt kennis van psychiatrie; ervaring met verslaving, ASS of LVB is een pré
  • Je staat stevig in je schoenen en kunt omgaan met complex en intensief gedrag
  • Je werkt zelfstandig en methodisch en denkt in oplossingen
  • Je kunt grenzen stellen én flexibel loslaten wanneer dat kan
  • Je houdt van pionieren en denkt actief mee over deze vernieuwende woonvorm
  • Je bent flexibel inzetbaar en draait alle diensten (24-uurszorg)
