Met jouw warme houding, coördinerende skills en scherpe blik zorg je voor een veilige, vertrouwde plek waar cliënten zich gezien en gesteund voelen. Van persoonlijke zorg tot kleine signalen oppikken, van samenwerken met verwanten tot het bewaken van rust in huis: jij maakt elke dag impact.

Wat je doet:

  • Opstellen, uitvoeren en evalueren van het ondersteuningsplansamen met de cliënt en zijn/haar netwerk
  • Signaleren van veranderingen in het welzijn of gedrag van cliënten en dit vertalen naar passende begeleiding
  • Coördineren van de zorg en afstemmen met collega’s, behandelaren en externe instanties
  • Begeleiden van cliënten in hun eigen appartement en ondersteunen bij dagelijkse zorg (ADL) en structuur
  • Cliënten stimuleren in hun zelfstandigheid en eigen regie, altijd vanuit hun wensen en mogelijkheden
  • Actief bijdragen aan teamoverleggen en de verdere ontwikkeling van de locatie Veldheem

Jij bent iemand die verantwoordelijkheid pakt en verbinding legt. Je combineert methodisch werken met een open, menselijke aanpak. Je houdt overzicht, durft keuzes te maken en schakelt makkelijk met familie en samenwerkingspartners. Tegelijkertijd weet je cliënten te motiveren en hun kracht naar boven te halen.

  • Je hebt een afgeronde mbo-4 of hbo-opleiding richting Zorg & Welzijn
  • Je neemt verantwoordelijkheid en bewaart overzicht, ook in complexe situaties
  • Je hebt ervaring of affiniteit met cliënten met een licht tot matig verstandelijke beperking, NAH en/of psychische problematiek
  • Je werkt methodisch en planmatig, maar blijft flexibel en cliëntgericht
  • Je communiceert gemakkelijk met cliënten, hun familie én samenwerkingspartners
