Als Persoonlijk Begeleider ben jij een vaste en vertrouwde kracht voor een aantal bewoners. Je bent hun eerste aanspreekpunt en verantwoordelijk voor het opstellen, uitvoeren en evalueren van het persoonlijk ondersteuningsplan. Samen met de bewoner en het netwerk kijk je naar wat nodig is om zo prettig en zelfstandig mogelijk te leven.

Je begeleidt methodisch en met aandacht voor structuur, veiligheid en welzijn. Je stimuleert eigen regie en ontwikkeling, signaleert veranderingen en onderneemt zelfstandig actie wanneer dat nodig is. Daarbij werk je nauw samen met collega’s, behandelaren en andere disciplines.

Daarnaast:

  • onderhoud je actief contact met verwanten, vrijwilligers en externe zorgverleners;
  • voer je regie op de begeleiding en zorg je voor goede afstemming binnen het team;
  • zorg je voor zorgvuldige rapportage en dossiervoering;
  • draag je bij aan een veilig, stabiel en warm woon- en leefklimaat;
  • draai je dag-, avond- en slaapdiensten en bied je continuïteit in de begeleiding.

Jij bent een betrokken en professionele begeleider die met een open blik kijkt naar wat wel kan. Je neemt verantwoordelijkheid voor je werk en voelt je medeverantwoordelijk voor de kwaliteit van zorg.

Verder:

  • heb je een afgeronde mbo-opleiding niveau 4 in de richting Zorg & Welzijn, Agogie of Verpleegkunde;
  • heb je affiniteit met de doelgroep en werk je graag vanuit eigen regie en ontwikkelingsgericht begeleiden;
  • communiceer je open, duidelijk en met aandacht, ook in complexere situaties;
  • kun je goed schakelen, prioriteiten stellen en blijf je rustig als het even anders loopt;
  • werk je zorgvuldig, ook als het gaat om rapportages en digitale systemen;
  • draag je actief bij aan teamontwikkeling en kwaliteitsverbetering;
