Als persoonlijk begeleider ben jij het aanspreekpunt van de zorg rondom jouw cliënten. Je stelt samen met de cliënt en het netwerk het ondersteuningsplan op, bewaakt de doelen en vertaalt dit naar de dagelijkse begeleiding. Je weet wat iemand nodig heeft om stappen te zetten en speelt daar continu op in. Met jouw scherpe blik en warme aanwezigheid voelt iedere cliënt zich gezien en gesteund.

Verder ga je:

• Ondersteunen bij dagelijkse activiteiten, structuur en persoonlijke doelen • Opstellen, uitvoeren en evalueren van het ondersteuningsplan • Coördineren van zorg en afstemmen met collega’s, behandelaren en externe betrokkenen • Signaleren van veranderingen in gedrag en dit vertalen naar passende begeleiding • Begeleiden volgens de ‘Geef me de Vijf’-methode en werken vanuit de beleving van de cliënt • Stimuleren van zelfstandigheid en eigen regie, passend bij de mogelijkheden van de cliënt • Contact onderhouden met familie en het netwerk • Actief bijdragen aan teamoverleggen en de ontwikkeling van de locatie • Werken in onregelmatige diensten (dag, avond en slaap), met ruimte voor zelfroosteren

Jij bent iemand die verantwoordelijkheid pakt en verbinding legt. Je combineert methodisch werken met een open, menselijke aanpak. Je houdt overzicht, durft keuzes te maken en schakelt makkelijk met familie en samenwerkingspartners. Tegelijkertijd weet je cliënten te motiveren en hun kracht naar boven te halen.

  • Je hebt een afgeronde mbo-4 of hbo-opleiding richting Zorg & Welzijn
  • Je neemt verantwoordelijkheid en bewaart overzicht, ook in complexe situaties
  • Je hebt ervaring of affiniteit met cliënten met een licht tot matig verstandelijke beperking, NAH en/of psychische problematiek
  • Je werkt methodisch en planmatig, maar blijft flexibel en cliëntgericht
  • Je communiceert gemakkelijk met cliënten, hun familie en samenwerkingspartners
