Als Persoonlijk Begeleider ben jij voor meerdere bewoners een belangrijk en vertrouwd aanspreekpunt. Je kijkt naar wat iemand nodig heeft, maar vooral ook naar wat iemand zelf kan, wil en nog kan leren.

Je bent verantwoordelijk voor het opstellen, uitvoeren en evalueren van het zorgplan. Samen met de bewoners, familie, het netwerk en andere betrokken professionals stel je doelen op en zorg je dat de begeleiding goed op elkaar aansluit. Je signaleert veranderingen, houdt het clientdossier en de rapportages op orde en schakelt waar nodig met andere disciplines.

Tegelijkertijd sta je midden in het dagelijks leven van onze bewoners. Je biedt structuur, stimuleert zelfstandigheid en sociale vaardigheden en ondersteunt waar dat nodig is. Afhankelijk van de locatie en doelgroep kan dat ook betekenen dat je ondersteunt bij ADL, verzorging, verpleegtechnische handelingen, activiteiten of dagbesteding.

Bij Careander kijken we verder dan alleen de zorgvraag. Wat kan iemand zelf? Wat kan iemand nog leren? Welke rol kan het eigen netwerk spelen? En wanneer is professionele begeleiding nodig? Zo werken we samen aan onze beweging van zorg naar gewoon leven.

Ook binnen je team neem je verantwoordelijkheid. Je deelt je kennis, denkt mee over hoe de begeleiding beter kan en kunt nieuwe collega’s, leerlingen of stagiaires begeleiden.

Zo pas jij perfect in deze rol

Je hebt een afgeronde mbo-4 opleiding in de richting agogie of verpleegkunde. Ervaring binnen de gehandicaptenzorg is mooi meegenomen, maar minstens zo belangrijk is dat je wilt begrijpen wat iemand nodig heeft en kijkt naar wat wél mogelijk is.

Je kunt goed observeren, verantwoordelijkheid nemen en zelfstandig beslissingen maken. Je communiceert makkelijk met bewoners, familie, collega’s en andere professionals en weet je communicatie aan te passen aan degene die tegenover je zit.

Ook wanneer een situatie ingewikkelder wordt, blijf je rustig en duidelijk. Je kunt motiveren, stimuleren en grenzen stellen en weet wanneer je collega’s, een gedragsdeskundige of teammanager moet inschakelen.

Geduld, inlevingsvermogen en doorzettingsvermogen zijn belangrijk in je werk. Net als nieuwsgierigheid. Want geen client is hetzelfde en dus blijf je leren.

En het mooie is: je hoeft vooraf niet zelf te weten welke Careander-locatie het beste bij jou past.
