The posting

Binnen het Volledig Pakket Thuis(VPT)‑team in Breukelen ondersteun je cliënten om zo lang mogelijk op een veilige en prettige manier thuis te blijven wonen. De zorgvragen worden steeds complexer en vragen om overzicht, regie en goede samenwerking. Als Regisseur VPT ben jij de verbindende schakel tussen cliënten, naasten, collega’s en andere betrokken zorgprofessionals. Met jouw ervaring en leiderschap zorg je voor samenhang, kwaliteit en continuïteit van zorg.

Dit ga je doen als Regisseur VPT

Je vervult een coördinerende rol binnen het team en zorgt ervoor dat cliënten de juiste zorg en ondersteuning ontvangen. Daarbij houd je het overzicht, stel je prioriteiten en zorg je dat iedereen vanuit zijn eigen verantwoordelijkheid bijdraagt aan passende zorg.

Concreet betekent dat:

  • Coördineren van zorg en ondersteuning voor cliënten die gebruikmaken van Volledig Pakket Thuis óf Modulair Pakket Thuis.
  • Opstellen, evalueren en bijstellen van zorgplannen in samenwerking met cliënten en hun naasten.
  • Regie voeren bij complexe zorgsituaties, met name rondom cliënten met dementie.
  • Afstemmen met huisartsen, behandelaren, wijkteams en andere ketenpartners.
  • Begeleiden en ondersteunen van collega’s en bijdragen aan een goede samenwerking binnen het team.

Wat je van ons mag verwachten

  • Salaris volgens cao VVT, FWG 50: € 3.592 - € 4.822 (o.b.v. 36 uur).
  • Direct een vast contract voor 24 tot 32 uur per week.
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.

Wat jij meebrengt

  • Een diploma Verpleegkunde op mbo‑ of hbo‑niveau.
  • Ervaring in de wijkverpleging en kennis van coördinatie van zorg.
  • Leiderschap en het vermogen om overzicht te bewaren in complexe situaties.
  • Sterke communicatieve vaardigheden en een verbindende werkstijl.
  • Zelfstandigheid, organisatiesensitiviteit en het vermogen om prioriteiten te stellen.

Over het team

Je komt terecht in een betrokken VPT‑team van ongeveer tien collega’s. Samen ondersteunen jullie cliënten met uiteenlopende zorgvragen, waarbij dementie een belangrijke doelgroep vormt. De sfeer binnen het team is collegiaal en praktisch; collega's weten elkaar te vinden en werken samen aan passende zorg voor cliënten thuis. Binnen deze rol krijg je veel ruimte om zelfstandig te werken en richting te geven aan de verdere ontwikkeling van het team.

Om de veiligheid van onze cliënten en medewerkers te waarborgen, is voor alle medewerkers die bij Careyn in dienst komen een Verklaring Omtrent Gedrag (VOG) vereist. Bij gelijke geschiktheid krijgen interne sollicitanten voorrang ten opzichte van externe kandidaten.
