Ben jij een psychosociaal medewerker met een hart voor herstelgerichte zorg? Werk je graag in een grootstedelijke, diverse context? Wil je mensen met een psychische kwetsbaarheid ondersteunen én bruggen bouwen in hun netwerk?

Dan zijn wij op zoek naar jou!

CGG Brussel vzw zoekt een psychosociaal medewerker voor het volwassenenteam van deelwerking Zuid (Vorst). Je versterkt cliënten in hun herstelproces, werkt verbindend en draagt bij aan toegankelijke, contextbewuste zorg op maat.

Jouw rol als psychosociaal medewerker

Je begeleidt volwassenen met een psychiatrische kwetsbaarheid in een ambulante context. Vanuit een herstelgerichte, outreachende en contextbewuste benadering werk je aan versterking van zelfredzaamheid en maatschappelijke participatie. Je bouwt bruggen tussen cliënten, hun netwerk én relevante partners in Brussel.

Je maakt deel uit van een multidisciplinair team (psychologen, psychiaters en een onthaalmedewerker) dat vertrouwd is met complexe problematieken en hulpvragen.

Je taken zijn o.a.:

  • Je begeleidt cliënten op maat, met aandacht voor herstel, zelfzorg en activatie.
  • Je biedt psycho-educatie en ondersteuning op het vlak van wonen, administratie, dagbesteding, etc.
  • Je bewaakt, samen met het multidisciplinair team, het zorgtraject van aanmelding tot afsluiting of doorverwijzing.
  • Je werkt samen met diverse partners in Brussel (o.a. huisartsen, wijkgezondheidscentra, OCMW, mobiele teams en straathoekwerk).
  • Je signaleert, samen met het multidisciplinair team, risico's (zoals suïcidaliteit, psychische ontregeling, uithuiszetting, ...) en onderneemt gepaste actie.
  • Je stemt af met cliënt, netwerk en team via overlegmomenten.
  • Je registreert helder en zorgvuldig in het elektronisch patiëntendossier (EPD).
  • Je ondersteunt bij administratieve vragen en leidt correct toe naar eerstelijnsdiensten indien nodig.
  • Je neemt actief deel aan teamoverleg, intervisie en wijkgerichte samenwerking.
  • Je draagt bij aan groepsaanbod of psycho-educatieve sessies.

Wat we zoeken in jou

  • Een professionele bachelor in een menswetenschappelijke richting, bij voorkeur maatschappelijk werk.
  • Ervaring binnen de geestelijke gezondheidszorg en inzicht in psychopathologie.
  • Een hoopvolle, motiverende houding, ook bij cliënten die moeilijk aansluiting vinden.
  • Creativiteit, doorzettingsvermogen en daadkracht in crisissituaties.
  • Zelfstandigheid én een teamgerichte houding.
  • Heldere, respectvolle communicatie.
  • Een cultuursensitieve en genderinclusieve basishouding, met affiniteit voor het Brusselse zorglandschap.
  • Vaardigheid in netwerken en samenwerken binnen een complex stedelijk zorgnetwerk.
  • Bereidheid tot avondwerk (1 avond/week tot 20u).
  • Een goede kennis van het Nederlands en een functionele kennis van het Frans is een must. Andere talen zijn een meerwaarde.
  • Een grondige kennis van de sociale kaart van Brussel of de bereidheid om je hierin snel in te werken.
