Wil jij als GGZ-agoog écht naast mensen staan en samen met hen werken aan herstel? Zoek je een ambulante functie waarin je veel zelfstandigheid hebt, geen dag hetzelfde is en je jouw expertise volop kunt inzetten? Dan is deze plek binnen het FACT-team in Zutphen misschien precies wat je zoekt.

Je werkt met volwassenen met ernstige en vaak complexe psychiatrische problematiek. Mensen die op meerdere gebieden in hun leven zijn vastgelopen en waarbij het soms even zoeken is naar de weg vooruit. Juist daar maak jij het verschil. Door aan te sluiten, vertrouwen op te bouwen, mogelijkheden te zien en samen kleine én grote stappen richting herstel te zetten.

Wat bieden wij jou?

  • Contract voor 24-36 uur per week, met intentie tot vast dienstverband
  • Salaris conform [job.schaal_+_salaris] bruto per maand (o.b.v. 36 uur p/wk)

Dit is je team

Je werkt vanuit Post Zutphen aan De Molengracht, binnen een multidisciplinair FACT-team met verschillende expertises. Je bent veel ambulant onderweg en werkt daar waar het voor de cliënt nodig is.

Je werkt nauw samen met collega’s én met huisartsen, gemeenten, familie en andere netwerkpartners. Daarmee ben je niet alleen begeleider, maar ook een belangrijke schakel rondom de cliënt. Je krijgt de ruimte om je eigen expertise in te zetten, mee te denken en jezelf verder te ontwikkelen.

Wat ga je doen?

Als GGZ-agoog bied je intensieve begeleiding aan volwassenen met ernstige psychiatrische aandoeningen en vaak meerdere problemen op verschillende levensgebieden. Je werkt herstelgericht en volgens de contextbenadering. Samen met de cliënt kijk je naar mogelijkheden, zelfredzaamheid en wat nodig is om weer grip te krijgen op het dagelijks leven.

Je bouwt een betekenisvolle werkrelatie op, signaleert veranderingen en weet goed om te gaan met complexe en soms onverwachte situaties. Je kijkt daarbij verder dan de diagnose: wat speelt er in iemands leven en wat kan bijdragen aan herstel?

Samen met de cliënt, diens netwerk en je collega’s stel je het herstelplan op en werk je aan de doelen. Soms betekent dat iemand helpen een praktische stap te zetten richting meer zelfstandigheid, soms juist snel schakelen wanneer een situatie verandert.

Juist die afwisseling maakt het werk interessant. Je bent dichtbij op momenten waarop het ertoe doet en kunt vanuit jouw expertise écht iets betekenen.

Dit vragen we van jou

  • Je hebt een afgeronde opleiding SPH of MWD, of misschien juist een diploma Social Work met het uitstroomprofiel Zorg. Heb je Toegepaste Psychologie gedaan? Dan ben je met de specialisatie tot GGZ-agoog én je inschrijving in het Register GGZ Agoog (Registerplein) ook van harte welkom!
  • Jij werkt graag in een ambulante setting en heb jij affiniteit met cliënten met meervoudige psychiatrische problematiek.
  • Jij bent in het bezit van een rijbewijs en eigen vervoer.

Als GGZ-agoog beschik jij daarnaast over goede sociale vaardigheden. Jij lost zelfstandig de knelpunten in de uitvoering van jouw eigen werkzaamheden op. Met jouw flexibiliteit zorg jij ervoor dat jij je kunt aanpassen aan de situatie en de hulpvraag van onze cliënten.
