The posting

Je werkt in zowel kortdurende als langer durende trajecten samen met cliënten om écht antwoord te geven op hun hulpvraag. De focus ligt vooral op de chronische en herstelgerichte doelgroep, met daarnaast ruimte voor kortdurende behandeltrajecten. Geen dag is hetzelfde: je schakelt tussen intensieve behandelingen, herstelondersteuning en het versterken van eigen regie bij cliënten. Je doet dit op verschillende locaties in en rondom Apeldoorn en Zutphen, zodat je zichtbaar en bereikbaar bent voor de mensen die jou nodig hebben.

Wat bieden we jou

  • Contract voor 24–36 uur per week, met intentie tot vast dienstverband
  • Salaris conform [job.schaal_+_salaris] bruto per maand (o.b.v. 36 uur p/wk)

Dit is je team

Je gaat werken bij Bas, een relatief jong, maar snelgroeiend onderdeel van GGNet. Bij Bas vinden we het belangrijk om laagdrempelig, dicht bij huis hulp aan te kunnen bieden waarbij je niet lang hoeft te wachten op een behandeling. We geloven in de veerkracht van mensen die om hulp vragen, hun omgeving, kortdurend behandelen en herstel!

We werken met het uitgangspunt om op twee verschillende locaties werkzaam te zijn, zo dicht mogelijk bij de patiënt. Deze locaties kunnen variëren van de bestaande GGNet locaties tot huisartsen- en wijkcentra.

Iedereen kan op een zeker moment in zijn leven met psychische problemen te maken krijgen. Sommigen voelen zich somber en hebben weinig energie. Anderen voelen zich angstig, hebben slaapproblemen of drinken te veel. Denk hierbij aan verslavingsproblematiek, schizofrenie, stemmingsstoornissen, LVB of persoonlijkheidsproblematiek.

Wat ga je doen?

Als verpleegkundig specialist behandel je vanuit de positieve gezondheid en methodische herstelgerichte wijze cliënten in hun eigen omgeving. Cognitieve gedragstherapie, het KOP-model en evidence-based behandelprotocollen staan centraal in je werk.

Je zet cliënten maximaal in eigen regie en maakt met hen blended-behandelplannen: gedeeltelijk gebaseerd op face-to-face contact (individueel/groepen) en gedeeltelijk gebaseerd op e-health-modules. Terugvalpreventie en farmacotherapie is ook een belangrijk onderdeel van het werk.

Enkele andere taken die je uitvoert zijn: • Regie behandelaar • Je kunt, als je dat wilt, ook optreden als aandachtsfunctionaris

✔Je bent in het bezit van het diploma Master Advanced Nursing Practice (MANP) of de GGZ-verpleegkundig specialist (GGZ-vs) en bent geregistreerd in het BIG-register (artikel 14 wet BIG) ✔Je bent ondernemend, toegankelijk, zelfstandig en energiek ✔Je bent in staat om collega´s enthousiast te maken en mee te nemen in de ontwikkelingen ✔Je wilt met e-Health werken en dit verder ontwikkelen
