The posting

Wil jij als casemanager echt verschil maken voor cliënten met een ernstig psychiatrische aandoening (EPA)? Voor één van onze FACT-teams zoeken wij een tijdelijke casemanager ter vervanging, voor de periode van een half jaar.

Als casemanager bij GGZ inGeest bied je cliënten structuur, begeleiding en continuïteit van zorg. Je observeert, signaleert veranderingen in fysieke en mentale gesteldheid en vertaalt dit naar passende acties binnen het behandelplan. Dat doe je niet alleen, maar samen met het multidisciplinair team van FACT Hoofddorp. Binnen dat team maken jouw observaties en regie het verschil.

Passende zorg voor jouw cliënten

  • De FACT ouderen teams richten zich op de behandeling en herstel van mensen met ernstige psychiatrische aandoeningen (EPA) boven de 65 jaar met ernstige problemen en disfunctioneren op meerdere levensdomeinen. Ook worden ouderen met cognitieve stoornissen, waar psychiatrische comorbiditeit bij speelt, behandeld. Indien nodig wordt de familie betrokken bij de behandeling.
  • FACT ouderen biedt langdurig zorgafhankelijke psychiatrische cliënten behandeling en steun op allerlei levensgebieden in de eigen omgeving buiten het ziekenhuis. Daardoor kunnen ook ernstig ontregelde chronisch psychiatrische cliënten met steun van multidisciplinaire FACT teams zelfstandig in de wijk (of in sommige gevallen in een RIBW) wonen.
  • Daarnaast ondersteunen we in het (weer) deelnemen aan de maatschappij. We werken daarin samen met andere organisaties om de cliënt hierbij zo goed mogelijk te ondersteunen. De cliënt krijgt extra hulp als het nodig is, minder hulp wanneer het kan.
  • Er wordt gewerkt volgens de F-ACT methode: een intensieve vorm van ambulante wijkgerichte behandeling vanuit herstelgericht oogpunt door verschillende mensen die in één team samenwerken. Ons uitgangspunt blijft om aansluiting te vinden bij de doelen en wensen van de cliënt.
  • Binnen FACT Ouderen wordt gewerkt in multidisciplinair teams bestaande uit psychiaters, specialist ouderengeneeskunde, verpleegkundig specialisten, gz-psychologen, basisartsen, ambulant verpleegkundigen/begeleiders, maatschappelijk werkers en trajectbegeleiders.

Jouw rol binnen het team

In nauwe samenwerking met collega-behandelaren neem je de regie over jouw eigen caseload. Dat betekent onder andere:

  • Huisbezoeken en afspraken op locatie
  • Intakegesprekken voeren
  • Behandelingen uitvoeren (systeemgericht en klachtgericht)
  • Signaleren en handelen in crisissituaties
  • Ondersteunen bij herstel en participatie in de maatschappij

Je bent empathisch en luisterbereid en hebt oog voor het volledige verhaal van de cliënt. Samen met cliënten en hun naasten werk je aan herstel, soms in kleine, maar betekenisvolle stappen.

Een belangrijk onderdeel van je werk is het onderhouden en uitbreiden van het netwerk rondom jouw cliënten. Je werkt samen met beschermd en begeleid wonen, welzijnsorganisaties, buurtteams, politie en woningcorporaties.

Wat breng je mee?

  • Een BIG-registratie als hbo-verpleegkundige óf
  • Een afgeronde opleiding hbo maatschappelijk werk & dienstverlening, hbo sociaal pedagogische hulpverlening of hbo social work (uitstroomprofiel Zorg) óf
  • Een afgeronde post-hbo opleiding tot GGZ-agoog

Heb je geen BIG-registratie? Dan gaan we graag met je in gesprek over het bevoegd en bekwaam raken in verpleegtechnische handelingen binnen FACT (injecteren en medicatietoediening).

Daarnaast:

  • Ben je in het bezit van rijbewijs en auto
  • Kun je zelfstandig werken én samenwerken in een multidisciplinair team
