Bij GGZ inGeest werk je als maatschappelijk werker in het FACT team Ouderen Amstelveen. Samen met een multidisciplinair team bied je herstelgerichte zorg aan cliënten met ernstige psychiatrische aandoeningen. Ontwikkel je verder in een ondersteunende werkomgeving en bouw mee aan de zorg van de toekomst. Je hart, kennis en ervaring zijn hard nodig.

Passende zorg voor jouw cliënten

  • De FACT ouderen teams richten zich op de behandeling en herstel van mensen met ernstige psychiatrische aandoeningen (EPA) boven de 65 jaar met ernstige problemen en disfunctioneren op meerdere levensdomeinen. Ook worden ouderen met cognitieve stoornissen, waar psychiatrische comorbiditeit bij speelt, behandeld. Indien nodig wordt de familie betrokken bij de behandeling.
  • FACT ouderen biedt langdurig zorgafhankelijke psychiatrische cliënten behandeling en steun op allerlei levensgebieden in de eigen omgeving buiten het ziekenhuis. Daardoor kunnen ook ernstig ontregelde chronisch psychiatrische cliënten met steun van multidisciplinaire FACT teams zelfstandig in de wijk (of in sommige gevallen in een RIBW) wonen.
  • Daarnaast ondersteunen we in het (weer) deelnemen aan de maatschappij. We werken daarin samen met andere organisaties om de cliënt hierbij zo goed mogelijk te ondersteunen. De cliënt krijgt extra hulp als het nodig is, minder hulp wanneer het kan.
  • Er wordt gewerkt volgens de F-ACT methode: een intensieve vorm van ambulante wijkgerichte behandeling vanuit herstelgericht oogpunt door verschillende mensen die in één team samenwerken. Ons uitgangspunt blijft om aansluiting te vinden bij de doelen en wensen van de cliënt.
  • Binnen FACT Ouderen wordt gewerkt in multidisciplinair teams bestaande uit psychiaters, specialist ouderengeneeskunde, verpleegkundig specialisten, gz-psychologen, basisartsen, ambulant verpleegkundigen/begeleiders, maatschappelijk werkers en trajectbegeleiders.

Passende werkomgeving voor jou

  • In nauwe samenwerking met collega-behandelaren neem je de regie over de cliënten in jouw caseload. Dit omvat onder meer huisbezoeken en afspraken met de cliënt op locatie, waarbij de focus ligt op het verbeteren van hun maatschappelijke participatie.
  • Je begeleidt en ondersteunt cliënten bij praktische en sociale hulpvragen, zoals financiële zaken, huisvesting en dagbesteding. Daarnaast signaleren je veranderingen in de situatie van de cliënt en speelt hier adequaat op in door samen met het team passende interventies in te zetten.
  • Je stimuleert de zelfredzaamheid en autonomie van cliënten door middel van motiverende gespreksvoering en oplossingsgerichte begeleiding en betrekt de familie en naasten bij het behandelproces om een ondersteunend netwerk te waarborgen.
  • Je bent empathisch en luisterbereid, waarbij je oog hebt voor het volledige verhaal van de cliënt. Samen met hen en hun naasten streef je naar herstel, hoe klein de stappen soms ook lijken.
  • Het actief samenwerken met ketenpartners zoals huisartsen, wijkverpleging, woningcorporaties en gemeentelijke instanties om de best mogelijke zorg te realiseren is een belangrijk onderdeel van je werkzaamheden.
  • Je draagt bij aan het opstellen en evalueren van behandelplannen in samenwerking met het FACT-team.
  • Voor maatschappelijk werkers, social workers, SPH’ers of GGZ-agogen die ervaring en/of affiniteit hebben met de doelgroep ouderen, houden van een uitdaging en op zoek zijn naar meer vrijheid in hun functie, is dit een uitgelezen kans: Je werkt in een gedreven en hecht team met veel eigen regie over je eigen agenda!

Ook belangrijk

  • Je hebt een afgeronde opleiding HBO maatschappelijk werk & dienstverlening, HBO sociaal pedagogische hulpverlening of HBO social work (uitstroomprofiel Zorg). Ook met een afgeronde post-HBO opleiding tot GGZ-agoog is het mogelijk om te solliciteren.
  • Je bent in het bezit van een auto en rijbewijs.
  • De functie is ingeschaald in FWG 55 cao GGZ.
