Als persoonlijk begeleider bij GGZ inGeest werk je op een nieuwe besloten woonafdeling met volwassenen met een langdurige en ernstige psychiatrische aandoening. Jij hebt een coördinerende rol rondom een aantal bewoners en bewaakt samen met het team het herstelproces.

Over de afdeling

Op Westerpoort openen we een besloten woonafdeling voor cliënten met een psychische kwetsbaarheid, vaak in combinatie met middelengebruik en ingrijpende levensgebeurtenissen. Veel bewoners verblijven hier langere tijd en hebben al meerdere behandeltrajecten doorlopen.

We werken herstelgericht volgens de ART-methodiek, waarbij we samen met bewoners doelen opstellen op verschillende levensgebieden. De afdeling is besloten: de deur is gesloten, maar kan open wanneer dat past binnen de gemaakte afspraken. Veiligheid, voorspelbaarheid en relatieopbouw staan centraal.

Je maakt deel uit van een nieuw team waarin je ook inhoudelijk richting geeft aan de opbouw van de afdeling. We zoeken collega’s die overzicht houden, duidelijk communiceren en regie durven nemen. De locatie ligt in een groene, rustige omgeving die bijdraagt aan herstel en ruimte biedt om even op adem te komen.

Wat ga je doen?

Als persoonlijk begeleider heb je regie op het traject van een aantal bewoners.

Je:

  • stelt samen met de bewoner doelen op binnen de ART-methodiek;
  • coördineert de begeleiding en stemt af met behandelaren en naasten;
  • bewaakt voortgang, evaluaties en rapportage;
  • ondersteunt collega’s inhoudelijk rondom jouw bewoners;
  • combineert begeleiding met observatie en signalering;
  • draagt bij aan de inhoudelijke ontwikkeling van de afdeling.

Wie ben jij?

Je bent iemand die overzicht houdt en verantwoordelijkheid durft te nemen. Je kunt schakelen tussen contact maken met bewoners en het bewaken van doelen, afspraken en voortgang.

Daarnaast:

  • werk je methodisch en herstelgericht;
  • neem je regie in het traject van bewoners en bewaak je de voortgang;
  • ben je communicatief sterk richting behandelaren, collega’s en naasten;
  • kun je prioriteren in complexe situaties;
  • geef je duidelijkheid en structuur, ook wanneer het spannend wordt;
  • draag je actief bij aan de inhoudelijke ontwikkeling van het team.

Wat breng je mee?

  • HBO-opleiding Social Work, MWD, verpleegkunde of vergelijkbaar;
  • Ervaring binnen GGZ, beschermd wonen of vergelijkbare setting;
  • Ervaring met coördineren van zorg of begeleidingstrajecten is een pré;
  • Je werkt herstelgericht en bent communicatief sterk.
