GGZ inGeest zoekt een specialist ouderengeneeskunde voor het FACT team Ouderen, regio Amstelland en Meerlanden. Als specialist ouderengeneeskunde biedt je cruciale zorg aan cliënten met cognitieve stoornissen en psychiatrische co-morbiditeit. De uitdaging ligt in het verder kijken naar mogelijkheden, om samen vanuit de herstelgedachte in kleine stappen te werken aan kwaliteit van leven met de cliënt en zijn naasten, met steun van ketenpartners.

Passende zorg voor jouw cliënten

  • De FACT oudere teams van GGZ inGeest richten zich op de behandeling en herstel van mensen met ernstige psychiatrische aandoeningen (EPA) boven de 65 jaar met ernstige problemen en disfunctioneren op meerdere levensdomeinen. Ook worden ouderen met cognitieve stoornissen, waar psychiatrische comorbiditeit bij speelt, behandeld. Indien nodig wordt de familie betrokken bij de behandeling.
  • FACT ouderen biedt langdurig zorgafhankelijke psychiatrische cliënten behandeling en steun op allerlei levensgebieden in de eigen omgeving buiten het ziekenhuis. Daardoor kunnen ook ernstig ontregelde chronisch psychiatrische cliënten met steun van multidisciplinaire FACT teams zelfstandig in de wijk (of in sommige gevallen in een RIBW) wonen.
  • Daarnaast ondersteunen we in het (weer) deelnemen aan de maatschappij. We werken daarin samen met andere organisaties om de cliënt hierbij zo goed mogelijk te ondersteunen. De cliënt krijgt extra hulp als het nodig is, minder hulp wanneer het kan.
  • Er wordt gewerkt volgens de F-ACT methode: een intensieve vorm van ambulante wijkgerichte behandeling vanuit herstelgericht oogpunt door verschillende mensen die in één team samenwerken. Ons uitgangspunt blijft om aansluiting te vinden bij de doelen en wensen van de cliënt.
  • Binnen FACT Ouderen wordt gewerkt in multidisciplinair teams bestaande uit een psychiater, specialist ouderengeneeskunde, gz-psycholoog, artsen in opleiding voor psychiater of specialist ouderengeneeskunde, ambulant verpleegkundigen, GGZ agogen en een ervaringsdeskundige.

Passende werkomgeving voor jou

  • Je taken bestaan uit het doen van diagnostiek, behandeling, en crisisinterventie bij de doelgroep ouderen met neuro-psychiatrische problematiek.
  • Je bent als regiebehandelaar eindverantwoordelijk voor de zorg aan deze doelgroep en voeg je in op vragen vanuit het team met betrekking tot somatiek, en meekijken als er ook sprake is van cognitieve problematiek bij deze doelgroep.
  • In het kader van de Wet Verplichte GGZ ben je betrokken bij het aanvragen van zorg machtigingen.
  • Het opzoeken en onderhouden van de samenwerking met de verpleeghuissector binnen het netwerk in de regio Amstelland is een belangrijk onderdeel van je functie.
  • Je geeft vorm aan de somatische screening conform de richtlijnen.
  • Binnen de twee andere FACT Ouderen teams van GGZ inGeest zijn nog twee specialisten ouderengeneeskunde werkzaam, en is er onderling samenwerking met betrekking tot de dagcreening.
