Word trajectbegeleider in een FACT-team waar jouw blik écht telt. Je begeleidt cliënten bij het vinden van passende dagbesteding: een plek waar zij structuur, eigenwaarde en perspectief kunnen opbouwen. Dat doe je samen met een multidisciplinair team van specialisten, verpleegkundigen en begeleiders. Jij staat dicht bij de cliënt, ziet wat er speelt en signaleert wat nodig is en precies die observaties maken binnen het team het verschil.

Passende zorg voor jouw cliënten

  • Het FACT team behandelt ouderen (vanaf 65 jaar) met een ernstig psychiatrische aandoening (EPA) en hulpvragen op verschillende levensgebieden, zoals wonen, , dagbesteding,/ maatschappelijke participatie zingeving, gezondheid, zelfredzaamheid, financiën en sociale contacten.
  • Er wordt gewerkt volgens de F-ACT-methode: een intensieve vorm van ambulante, wijkgerichte behandeling vanuit een herstelgericht oogpunt door verschillende mensen die in één team samenwerken. Behandelingen vinden zo veel als mogelijk plaats in de eigen omgeving van de cliënt.
  • Er wordt altijd actief gezocht naar de samenwerking met de cliënt, diens naasten en ketenpartners. Ons uitgangspunt blijft om aansluiting te vinden bij de doelen en wensen van de cliënt.
  • Binnen FACT wordt gewerkt in multidisciplinair teams bestaande uit psychiaters, verpleegkundig specialisten, gz-psychologen, basisartsen, ambulant verpleegkundigen/begeleiders, maatschappelijk werkers en trajectbegeleiders. Het team heeft korte lijnen met onze ketenpartners en collega's van andere afdelingen binnen GGZ inGeest.

Passende werkomgeving voor jou

  • Als trajectbegeleider maak je deel uit van het behandelteam en heb je daarnaast nauwe contacten met andere organisaties zoals welzijnsorganisaties.
  • Je pakt vragen op uit het team met betrekking tot vragen over de wens van cliënten naar dagbesteding, uitbreiding sociaal netwerk en het vergroten van zelfredzaamheid. Daarnaast doe je op verzoek ergotherapeutisch onderzoek.
  • Je bezoekt cliënten thuis of spreekt af op een plek in de wijk die zij prettig vinden of helpend is bij het opbouwen van activiteiten buitenshuis. Samen onderzoeken jullie de hulpvraag en stel je een plan van aanpak op om hun doelen te bereiken. Je hebt oprechte aandacht voor iedere cliënt, terwijl je tegelijkertijd een professionele houding behoudt. De eigen regie van de cliënt staat bij jou centraal en je sluit aan bij hun behoeften.
  • Je bent altijd actief op zoek naar plekken waar cliënten zich thuis voelen en kunnen deelnemen aan activiteiten. Je bouwt een netwerk op van buurthuizen en welzijnsorganisaties, zodat je cliënten makkelijk van dagbesteding kan voorzien.

Ook belangrijk

  • Je hebt een afgeronde HBO-opleiding die valt binnen de ZPM beroepenstructuur: SPH, Maatschappelijk werk en dienstverlening, Social Work (Afstudeerrichting Zorg), of een post-HBO opleiding tot GGZ-Agoog, een afgeronde HBO opleiding tot ergotherapeut.
  • Je hebt een rijbewijs.
  • De functie is ingeschaald in FWG 50 cao GGZ.
