Jij zorgt ervoor. Zo werk jij Als coördinerend begeleider ben jij er voor ouderen met een complexe psychiatrische aandoening, vaak gecombineerd met somatische en cognitieve problematiek. Je biedt dagelijkse begeleiding en ondersteunt bij het behouden van vaardigheden en regie, en je creëert een veilige en voorspelbare leefomgeving waarin cliënten zich gezien voelen. Daarnaast werk je overstijgend over de verschillende afdelingen binnen Eikenstaete. Vanuit jouw expertise en persoonlijke talenten geef jij vorm aan de functie van begeleider. De belangrijkste taken en verantwoordelijkheden zijn voor jou bekend terrein. Wat de functie bij GGz Centraal bijzonder maakt is: - het bieden van intensieve begeleiding en verzorging, waarbij ADL-zorg een belangrijk en structureel onderdeel is van je werk. - het werken in een 24-uurs setting, waarbij je dag-, avond- en nachtdiensten draait en actief bijdraagt aan continuïteit van zorg. - het signaleren van veranderingen in gedrag en gezondheid en hier proactief op inspelen. - het samenwerken in een multidisciplinair team en bijdragen aan het uitvoeren van behandel- en begeleidingsplannen. - het vervullen van een regierol binnen het team, waarbij je als 'coördinerend begeleider' de ART-methodiek (Active Recovery Triad) actief helpt implementeren en verder uitrolt binnen Eikenstaete, met hiervoor ook boventallige tijd naast je reguliere diensten op Eikenstaete B.
