Consulent zorg en veiligheid en OGGZ-coördinatie Detachering Sociaal Domein 0-3 jaar Hbo Groningen 32 - 40 uur €3824 - €5624 Als Adviseur OGGZ maak jij het verschil voor mensen in een kwetsbare situatie in de gemeente Westerkwartier. Dit ga je doen Wil jij jouw expertise binnen de geestelijke gezondheidszorg inzetten om mensen in kwetsbare situaties verder te helpen? Ben jij sterk in analyseren, triageren en samenwerken met verschillende ketenpartners? Dan is deze opdracht binnen de Openbare Geestelijke Gezondheidszorg (OGGZ) bij de gemeente Westerkwartier iets voor jou! Er is ruimte ontstaan van 1 november a.s. t/m 1 februari a.s., met optie tot 3 x 3 maanden verlenging. Het aantal uren betreft 36 uur/week. Een contract bij Kenonz sluit aan bij deze periodes, een contract voor onbepaalde tijd is bespreekbaar. Wat ga je doen? Als professional binnen de OGGZ speel je een belangrijke rol in het beoordelen van meldingen en het signaleren van complexe zorg- en veiligheidsvraagstukken. Je brengt situaties zorgvuldig in kaart, voert triages en risicobeoordelingen uit en bepaalt welke organisatie verantwoordelijk is voor het vervolgtraject. Daarbij houd je oog voor zowel de persoonlijke situatie van betrokkenen als de bredere belangen op het gebied van zorg en veiligheid. Je werkt nauw samen met diverse ketenpartners, waaronder gemeenten, politie, woningcorporaties en zorginstellingen. Door informatie zorgvuldig te analyseren en helder te rapporteren, draag je bij aan een passende opvolging van meldingen en een effectieve samenwerking tussen betrokken organisaties. Daarnaast denk je actief mee over het verbeteren van werkprocessen, protocollen en handboeken. Je neemt deel aan overleggen en projectgroepen en verzorgt voorlichting over de werkzaamheden van de OGGZ.
