Met jouw expertise draag jij bij aan de uitstekende (ouderen)zorg die Rivas Zorggroep dagelijks levert. Dat is waarom jij dit werk wilt doen! Wat ga je doen? Als Physician Assistant/Verpleegkundig Specialist ouderengeneeskunde ben je verantwoordelijk voor de bevordering van gezondheid op het gebied van ouderenzorg. Je werkt nauw samen met de Specialisten Ouderengeneeskunde en houdt intercollegiaal overleg als het gaat om complexe ouderenzorg. Je bent verbindend en stelt kwaliteit binnen de ouderenzorg centraal. - Je werkt als regiebehandelaar, je gaat een zelfstandige behandelrelatie aan en combineert geprotocolleerde medische zorg met aandacht voor preventie, leefstijlmanagement en ondersteuning en coaching - Je coördineert het zorgproces van de cliënt en draagt zorg voor afstemming met de cliënt, familie en alle betrokken disciplines. - Je toont leiderschap in het optimaliseren van de kwaliteit van zorg en je geeft scholing aan zorgverleners op de nodige ontwikkelgebieden. - Je past de laatste wetenschappelijke inzichten toe in de praktijk en/of draagt bij aan wetenschappelijk onderzoek. Waar ga je werken? De vakgroep ouderengeneeskunde bestaat uit Specialisten ouderengeneeskunde, een aantal ANIOS, verpleegkundig specialisten en Physician assistants.
