Ben jij een betrokken en creatieve Klantcoach Welzijn die graag het verschil maakt voor cliënten? In deze veelzijdige functie werk je binnen een combinatie van Dag Activiteiten Centra (DAC) Herman Gorterhof en DAC ’t Schip, waar persoonlijke aandacht, ontwikkeling en welzijn centraal staan. Je ondersteunt cliënten op sociaal, emotioneel en praktisch vlak en helpt hen om betekenisvolle momenten in hun dag te ervaren. Samen met collega’s, vrijwilligers en mantelzorgers zorg je voor een warme en stimulerende omgeving waarin iedereen zich gezien en gewaardeerd voelt.

Dit ga je doen als Klantcoach

Als Klantcoach Welzijn ondersteun en begeleid je onze cliënten op sociaal, emotioneel en praktisch vlak. Je werkt vanuit het gedachtengoed van Positieve Gezondheid en zoekt samen met de cliënten naar wat het leven voor hen betekenisvol maakt. Daarbij werk je nauw samen met collega’s, mantelzorgers, stagiaires en vrijwilligers binnen een warm en respectvol team. Je komt te werken op onze bewoners met psychogeriatrische (PG) problematiek wonen.

Concreet betekent dat:

  • Samen ontdekken wat bewoners belangrijk vinden en hoe ze hun dagen graag invullen.
  • Bewoners stimuleren om actief mee te doen en contact te houden met hun omgeving.
  • Zelf activiteiten bedenken of aansluiten bij wat er al is - van koffiemoment tot spelletjes tot het organiseren van activiteiten op de afdeling en/ of in het Domein, waar bewoners samen genieten van ontmoeting en gezelligheid.
  • Een belangrijke schakel zijn tussen bewoners, hun familie en collega’s.
  • Samenwerken met zorgmedewerkers om zorg en activiteiten zo goed mogelijk op elkaar aan te laten sluiten, mede door rapportages in zorgdossiers.

Wat je van ons mag verwachten

  • Salaris volgens cao VVT, FWG 35: minimaal € 2.663,54 tot maximaal € 3.592,96 (o.b.v. 36 uur per week).
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.

Wat jij meebrengt

  • Een diploma Sociaal Pedagogisch Werker (niveau 4), Activiteiten begeleider (niveau 4) of Medewerker Maatschappelijke zorg (niveau 3) aangevuld met relevante werkervaring.
  • Affiniteit met de doelgroep.
  • Creativiteit in het vinden van oplossingen.
  • Zelfstandigheid en flexibiliteit
  • Kennis van Word en Excel en ervaring met elektronische zorgdossiers.
  • Ervaring met het ontwikkelen van activiteiten en het coachen van vrijwilligers.

Om de veiligheid van onze cliënten en medewerkers te waarborgen, is voor alle medewerkers die bij Careyn in dienst komen een Verklaring Omtrent Gedrag (VOG) vereist. Bij gelijke geschiktheid krijgen interne sollicitanten voorrang ten opzichte van externe kandidaten.
