Werken in de wijkteams Pijnacker-Nootdorp-Delfgauw is werken in een dorpse, groene omgeving in de nabijheid van grote steden. De 2 wijkteams werken vanuit de teampost in hartje Pijnacker. Je speelt een belangrijke rol in het ondersteunen van cliënten thuis, met oog voor hun wensen en mogelijkheden. Daarbij kun je rekenen op de kracht van een professioneel en betrokken team.

Dit ga je doen als mbo Verpleegkundige

Als Verpleegkundige in de wijk ben je de spin in het web rondom de zorg van de cliënten in hun thuisomgeving. Je houdt je cliënten scherp in de gaten en denkt proactief en preventief, vooral in situaties die snel veranderen. Je onderhoudt nauw contact met de huisarts, betrekt familie en mantelzorgers actief bij de zorg en bent de steun en toeverlaat voor de wijkverpleegkundige. Je documenteert de geleverde zorg accuraat in het zorgsysteem, geeft heldere voorlichting en advies aan cliënten en hun omgeving over zorg en welzijn en draagt actief bij aan de teamontwikkeling en de continue verbetering van zorgprocessen.

  • Salaris volgens cao VVT, FWG 45: € 3.502,45 - € 4.245,84 (o.b.v. 36 uur per week).
  • Direct een vast contract voor 12 tot 28 uur per week.
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.
