The posting

Langer Zorg en Ondersteuning Thuis (LZO) is het concept waarbij Careyn met aandacht en tijd zorg en welzijn biedt aan ouderen die (langer) thuis willen blijven wonen. Dit doen wij ook voor inwoners van Maassluis en omgeving. Men krijgt in het eigen huis ondersteuning op het gebied van welzijn en zorg. Samen met de cliënt en mantelzorger borgen we dat men zo lang mogelijk op een fijne manier actief thuis kan blijven wonen.

Dit ga je doen als Regisseur

Je begeleidt cliënten en hun naasten bij complexe situaties waarin meerdere vormen van zorg en ondersteuning samenkomen. Vanuit jouw rol zorg je voor samenhang, duidelijke regie en een passende inzet van zorg en netwerk.

Concreet betekent dat:

  • Opstellen en bijstellen van zorgplannen op basis van klinisch redeneren en de ontwikkeling van de cliënt.
  • Regie voeren over complexe zorgsituaties en coördineren van de inzet van zorg en ondersteuning.
  • Begeleiden van cliënten en hun netwerk, met aandacht voor zelfregie en kwaliteit van leven.
  • Afstemmen met huisartsen, behandelaren en andere ketenpartners in de wijk.
  • Instrueren en ondersteunen van collega’s en bijdragen aan professionele werkwijzen binnen het team.

Wat je van ons mag verwachten

  • Salaris volgens cao VVT, FWG 50: € 3.592,96 - € 4.822,50 (o.b.v. 36 uur per week).
  • Direct een vast contract voor 24 tot 28 uur per week.
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.

Wat jij meebrengt

  • Een diploma Verpleegkunde (mbo-4, hbo) en een geldige BIG-registratie.
  • Kennis van indicatiestelling en ervaring met coördineren van zorg in de wijk.
  • Affiniteit met complexe zorgvragen, waaronder dementie.
  • Vaardigheid in het plannen, waarbij je prioriteiten weet te stellen.
  • Vermogen om zelfstandig beslissingen te nemen en overzicht te houden.
  • Een verbindende houding en overtuigingskracht in samenwerking met cliënten en professionals.

Om de veiligheid van onze cliënten en medewerkers te waarborgen, is voor alle medewerkers die bij Careyn in dienst komen een Verklaring Omtrent Gedrag (VOG) vereist. Bij gelijke geschiktheid krijgen interne sollicitanten voorrang ten opzichte van externe kandidaten.
