Dagelijks het verschil maken in het leven van ouderen die tijdelijk of langdurig ondersteuning nodig hebben. Binnen de teams Thuisbegeleiding van Careyn Utrecht Stad draait het om nabijheid, vertrouwen en structuur. De begeleiding vindt plaats bij mensen thuis, juist op momenten waarop het leven complex is geworden. Denk aan situaties waarin psychiatrie, verslaving of dementie samenkomen. Deze functie biedt ruimte voor zelfstandigheid, vakmanschap en betekenisvol werk, met altijd een deskundig team op de achtergrond.

Dit ga je doen als Thuisbegeleider

Binnen de thuisbegeleiding staat het herstel van regie en overzicht centraal. De begeleiding sluit aan op wat iemand nodig heeft om het dagelijks leven weer hanteerbaar te maken, met oog voor het tempo en de mogelijkheden van de cliënt.

Concreet betekent dat:

  • Aanbrengen van structuur in het dagelijks leven bij cliënten met meervoudige problematiek.
  • Vergroten van zelfredzaamheid en versterken van de eigen kracht van de cliënt.
  • Ondersteunen bij het opbouwen of herstellen van sociale netwerken en maatschappelijke participatie.
  • Afstemmen en samenwerken met de Buurtteams van de gemeente Utrecht.
  • Methodisch en zelfstandig bieden van begeleiding op maat in de thuissituatie.

Wat je van ons mag verwachten

  • Salaris volgens cao VVT, FWG 40: € 2.981 - € 3.864 (o.b.v. 36 uur).
  • Direct een vast contract voor 20 tot 32 uur per week.
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.

Wat jij meebrengt

  • Minimaal een mbo niveau 4 diploma, zoals Social Work, MMZ, SPW of vergelijkbaar, of een aantal deelcertificaten daarvan.
  • Generalistische inzet met kennis van psychiatrische problematiek, verstandelijke beperking, verslaving en dementie.
  • Ervaring met methodisch werken, zorgplannen en zelfstandig begeleiden.
  • Rijbewijs en beschikking over eigen vervoer.
